Print and sign two copies of each these forms. If you have been offered a membership (apply first), you'll need to sign both the Studio Policy Agreement and the Waiver to activate it. Access to the studio cannot be granted without receipt of these forms, in person. You and the studio will each retain a copy of signed agreements and waivers. Bring these signed forms with you on your move-in date, to be scheduled with our director.
